Supporting individuals with disabilities, their families, and caregivers as they navigate the transition from school to adult life.
Are you the parent or caregiver of a young person with disabilities? Are you concerned about that young person’s future and what life will look like for them after they leave school? Are you aware that you should begin to think about transition much earlier than the high school years?
Successful transition planning takes into consideration three major areas:
Transition Planning in the school: Is the school doing effective transition planning that addresses goals in employment, further education or vocational training, and independent living for your young person?
Government benefits: Have you been informed about potential government benefits that your young person may be eligible for, and have you started the application process?
Future planning: Are you aware of the need to do future financial and legal planning for your young person? Do you understand what will change when they turn 18, or reach the age of majority in your state? When they turn 21 or leave school?
New Visions Transition Consulting will guide you and your young person through the transition process and provide clarity and direction for each of these areas.
